Location: Children's Health- Dallas
Shift: Monday through Friday 8:00a to 5:00p
Job Type: Remote, but will be required to go onsite twice a month at Trinity Towers

Summary:
Provide family centered disease-specific education and care coordination for program participants from enrollment into the program until completion, through established process. Responsibilities include coordinating telephone contacts to provide developmentally appropriate education. Collaborates among the health care team internally and externally to provide care, referrals and support as necessary. responsible for the collection of clinical data for research used to evaluate quality of care, resource utilization and patient outcomes of the program. Assist with and participate in outreach and advocacy opportunities to increase awareness of disease state.

Responsibilities:
  • Care Coordination and Telephonic patient Contact: Dependent upon area, but this scope may include limited clinical activities not to exceed six hours per week. Telephonic interactions with patients, physicians, and payers. Care Coordination with regard to managing, referring physicians, payer issues, equipment and medication issues. Coordination of efforts with community agencies, school nurse, day care worker.
  • Data Collection and Monitoring: Responsible for outcome trending, data collection, data input, and basic calculation (mean).
  • Community Involvement and Advocacy: health fairs, workshops, speaking engagements and appropriate professional organization involvement.
How You’ll Be Successful:
WORK EXPERIENCE
  • At least 4 years related experience Required
  • At least 5 years Clinical Expertise, 2 years in sub-specialty areas Preferred
  • Telephone Triage experience Preferred
EDUCATION
  • Four-year Bachelor's degree or equivalent experience BSN Required
LICENSES AND CERTIFICATIONS
  • Current license to practice professional nursing in the State of Texas Required
  • Specialty area certification (AE-C or CDCES) 1 Year Required
  • Basic Life Support for Healthcare Providers as required by CP 1.20 Life Support Course Requirements Preferred
